In filling out this application from, I understand that C-K Composites Co, LLC (hereafter “the Company”) may wish to investigate any of the facts or statements submitted by me. I hereby grant permission to check any of this information except where my written statement upon this form specifically requests that no investigation to e made. (This includes reference checks with my present employers.)

I also understand that falsification, deliberate omission, or misrepresentation of facts upon this application will be grounds for or will be considered just cause for dismissal at the discretion of the Company should I become an employee of the Company.

I understand that the Company does not intend to alter, by this application, the traditional rule that either the Company or its employees may terminate the relationship at any time. I also understand the Company does not intend to create an implied contract between the Company and its employees through development and dissemination of Company policies, procedures, handbooks, or other literature. No contract with any employee is valid unless in writing signed by the President of the Company.

In addition, I understand that it is the policy of the Company to require that new hire or rehired employees complete a pre-employment, post offer medical examination and appraisal approved by the Company as a condition of employment. I understand that if conditions are detected which require further evaluation or review, or accommodation to ensure my capacity to successfully perform assigned duties with or without a reasonable accommodation, my application may be delayed or denied.
